I find it nearly impossible to hold the book up at a ninety degree angle with just your hands and flip the pages like Paul said. Using my thumb and index finger like that brakes my state because the book tilts in all directions because my left thumb and index finger are now having to help turn pages at the bottom, and I use them to support the book.

To tell you the truth, I don't hold the book up with my hands. I prop it up against another book at a 45 degree angle, (90 degree angle to your eyes) and just worry about my right-hand page turning technique. It's on the desk anyway, so why shouldn't I? Whatever helps keep me in the proper state of mind is whatever I should use, don't you think?
