I attended a training session with Izzy Katzeff. He twisted the spine of the book first. The top twisting to the right, the bottom twisted to the left. This caused a "step" exposing the edge of the pages towards the front of the book. Also he uses the thumb to press down on the book as he pushes the thumb diagonally towards the top right hand corner of the book. This pressure seems to bow the page to lift from the rest of the pages enabling you to "catch" the top page.
