I use a Rotring Drawing Pen with a permanent black ink (won't bleed all over the page when I use felt tips over it). Its a really fine (0.35mm) pen - quite expensive but worth it. I generally use it to do the text and drawings/symbols and then add colour as I go around the mind map. I normally just use laser/inkjet A4 paper - it seems OK to me and cheap (offset the pen!) and is easy to file away for later revision.
Ramon: Try toy stores or art stores for the multi-colour ballpoints - 'serious' stationers don't seem to stock them.
