If you find yourself itchy do what you normally do, you are more likely to remain in the accelerative learning state scratching it than trying to ignore it. Being in the accelerative learning state isn't a battle of the will it's just something you're doing to stay focused. You'll find once you've scratched the itch a few times it is less likely to bother you in the future.
No need to deliberately blurr your vision to avoid seeing words. You get pulled out when you 'stay' focused and start regular reading. You come out of the accelerative learning state slowly when you remain focused. Turn the book upside down if it you find yourself getting pulled into regular reading when you happen to 'notice' one word in focus.
Noticing a word here or there is OK... you know you've been thrown out of accelerative learning state when you find yourself regular reading rather than photoreading. 
Just maintain the divergent gaze while photoreading, say the chant (it helps you to maintain accelerative learning state and avoid getting caught up in reading) and when finished, close with a sense of mastery. The more books you photoread this way the easier it is to remain in the accelerative learning state.
