It is generally advised for large complex/technical books that each chapter be read as a seperate book.
You would then do all the steps on each chapter, it seems like a lot more work but you are needing that level of retention so you would want to do that.
The questions and triggers you write down have no limits, obviously you wouldn't want 100 of each but you want enough to intrigue your mind and make you "think".
When I'm ph'ing, I don't focus on the blip I just stare at the center seam and just go into photofocus, I turn the pages but I don't move my eyes to follow the page I just keep them in the center. This helps me regai n my focus quickly.
