Regarding your questions... You can certainly work on someone whether they are coordinated with your intentions, location, or not. I find the best results when the friend I am helping is aware of the time I am doing it. They may go lie down and relax, or perhaps relax in a meditative position. With people I have done a fair amount of work with, there is a somewhat heightened sense of that meeting and shared intention. Receptiveness and trust is wonderful. Great to proceed with the work at that point.
My experience has been, that if I 'reread' the energy, trying to sense blockages in an area more than once, I start to lose clearity. Almost as if a speck of doubt tells you to look again, or try to feel something more significant, or repeating it to find 'your' way of detecting, or sort of overworking it to learn about yourself as healer too. I guess the most helpful direction, is to do it like you know what you are doing, stay focused, keep it moving. Reflection, 'after' the session. Efficiency and a purity, during the process. All this said, if someone has a big blockage, I may find myself engaged with that longer, both in the sensing, clearing, and in the putting in side of equation.
The spine and the five organs are the nuts and bolts to clear. That is a simple and effective roadmap. Remember that everything is connected, and working with the five organs and spine connects to everything ' vital'. The spleen is important, 'and' part of the stomach system. If you find a specific blockage there, fine. If you take a general approach to 'areas', like midsection, or upper or lower body, or upper or lower torso,,,,all these approaches yield results too. Specific, or not so specific. Each time can be, and usually is, a bit different, and is the way it is.
Doing the level II active exercises helps in regards to arm fatigue, like sword finger and thunderpalm exercise. There is something sometimes referred to as a "bridge" in the arms. It pretains to holding up your arms. Doing the exercises, builds the strength of that bridge. Holding up your arms becomes more and more an experience of being filled or bouyed with energy, as opposed to a muscular, or tired muscular sensation.
If you do tire, remain aware of your breathing in of spiritual energy(ohm), and that energy coming through you to your friend, to stay engaged and focused as you rest your hand a bit.
