The main issue here is not so much the affirmations part of it. It's the resistance and blocks and negative self-talk that comes up whenever you try to change yourself.
Once you let go of the subconscious resistance to getting well, the rest is easy. People always try to work on one but not the other so they rarely succeed. The two opposing forces work against each other and may cancel each other out, leaving you with no progress at all.
Fist off, identify your negative self-talk, doubts, fears, pessimism, and resistance by writing each of them down on paper. Then work on each one by releasing them. Techniques such as the Sedona Method or EFT can help you release all that garbage. But once it's gone, your affirmations can proceed unopposed and be far more effective than if they had to fight an uphill battle.
Only then can you work on your affirmations, and you should verbalize them, mentalize them, visualize them, read them, write them, constantly all the time for a month or two.

The Sedona Method is the simplest of all but it costs money. One site on the web published it for free once but not sure if they're around anymore.
Most illnesses are caused by past emotional trauma and accumulated stresses which build up over time. If you identify all of the events in your past life and release each of them, it will work wonders for you. Write down a list of a few hundred or so of these events and work on each of them. Expect to take a few months to go through this process.
Another suggestion is to practice a meditative qigong system that will balance your emotions and internal organs to achieve health on a holistic level. For $40 you can get 3 CDs with 4 meditations but you can get by with the first two meditations. They're actually visualizations and their worth has been scientifcally validated in the field of psychoneuroimmunlogy. But you get the qigong benefits here that you won't get from other visualization/meditation programs by western hypnotherapists. Check out http://www.wishus.org/ It's a fantastic investment considering that other qigong purveyors normally charge two to three hundred dollars for this type of material.
All the best!
