I have difficulty focusing on task at hand, my mind is never present. When ever I am doing something, I feel like doing something else.

I just can't mentally exert myself, even things that I believe are well with my intellectual capacty.

Push yourself everyday to do more than the last intellectually, changes might be small gradually, but when you look back over the long term changes are bigger.

Holosync is also a good way of lifting the upper limit of the stress one is able to tolerate. My reading and 'concentration-tolerance' improved after a little Holosync.

Be patient with yourself, and praise yourself for any improvements.

What I found is while doing the paraliminals. I affirm to myself that now is the time that is chosen to do this and that there will be plenty of time after for other thinking. And after that realization I seem to stay with the paraliminal my mind dosn't wonder then. The other thing I do is let the world take care of itself while I do a paraliminal both distractions disappear for me.That little technique lets me stay in the moment.
Also this tecnique is applied to areas of my life where I want to put effort.

I'd put the Personal Genius Paraliminal to use. Use that with the tangerine technique on whatever you feel you need to focus on. I find just thinking of the tangerine brings my mind more onto the work at hand and less on the outside distractions.
