Should you concentrate on what each voice is saying and then once the voices start mixing together and talking at the same time should u be trying to take in what both voices are saying, or concentrate on the right channel and then next listening concentrate on the left channel?

Listen down the middle... means listen to neither directly. Don't force it. When you notice yourself hearing instructions in one ear follow that if you swap fine if you're not really following either, that's fine too. You intuitively will follow exactly what you need to at any particular time. It's like hearing your name in a crowded room, your attention is drawn to it soone enough.
