Perhaps you could use the smoke to reveal whats healthy underneath. That is turn the hernia into smoke and visualising a healthy vein instead. Perhaps underneath could be a play with time, perhaps the hernia was once a healthy vein, and you could play with turning the time that has passed since then into smoke. Time might not be such a linear thing after all, i dont know, but it might help you with your smoke problem thinking about the past, present and future as coexisting.
Same with incomplete bone tissue. Turn the incompleteness into smoke to reveal the healthy bone underneath. Again perhaps time might be of help. Maybe letting the smoke turn back time. Bringing the healthy bone back trough time. Maybe its easier to turn past health reality into future health reality, with the smoke being the sign of change.
